Schools and colleges can assist effective individual transition planning by:
- Nominating key staff who will be central to the individual transition planning team, including coordinator and a member of senior staff;
- Convening and coordinating transition planning meetings;
- Providing time and commitment to supporting the transition process;
- Involving young people and their parents/guardian in meaningful ways;
- Providing information in a medium accessible to young people and their parents or guardian;
- Encouraging and supporting effective communication and collaboration with all agencies and services involved;
- Providing school and community-based learning experiences that are relevant to the needs of the individual; and
- Linking the transition plan with any other plans for the young person.
