Transition planning for students with a disability

My Future Let's Plan has been developed to assist young people with a disability as they move into adulthood. It is based on sound theory and promising practice. With the young person central to all processes and practices, it includes policies, programs, practices, activities and experiences to prepare and support young people to move through school to post-school life, as well as theoretical and background information.

Guidelines for schools and colleges: Guidelines that schools and colleges can use to assist with individual transition planning.
Involving parents/guardians: Describes ways to involve parents/guardians in the transition planning process. Includes fact sheets and questionnaires.
Involving young people: Describes ways to involve young people in the transition planning process. Includes fact sheets.
Pathways: Describes a number of pathways students can take when moving from compulsory education. Includes information on available options, programs, courses, services and supports.
Processes and practices: Characteristics of effective transition planning.
Templates and fact sheets: Fact sheets and templates for use by young people and educators as well as by parents and guardians.
Timeline of activities: A suggested timeline of actions that young people, parents, Departmental personnel and others may wish to consider when planning for the future.
Resources: Transition planning resources including: employment, further education and training, references and websites, reports, policies and strategies, services, agencies and programs, funding, and teaching and learning.
