Transition planning for students with a disability

My Future Let's Plan has been developed to assist young people with a disability as they move into adulthood. It is based on sound theory and promising practice. With the young person central to all processes and practices, it includes policies, programs, practices, activities and experiences to prepare and support young people to move through school to post-school life, as well as theoretical and background information.
