Guidelines for schools and colleges

Schools and colleges can assist effective individual transition planning by:

  • Nominating key staff who will be central to the individual transition planning team, including coordinator and a member of senior staff;
  • Convening and coordinating transition planning meetings;
  • Providing time and commitment to supporting the transition process;
  • Involving young people and their parents/guardian in meaningful ways;
  • Providing information in a medium accessible to young people and their parents or guardian;
  • Encouraging and supporting effective communication and collaboration with all agencies and services involved;
  • Providing school and community-based learning experiences that are relevant to the needs of the individual; and
  • Linking the transition plan with any other plans for the young person.
