Templates and fact sheets

Fact sheets and templates for use by young people and educators as well as by parents and guardians.

Because transition planning is individualised, flexible and responsive to individual needs and circumstances, it is important to customise templates to reflect individual needs and circumstances. Some young people will benefit from using photos, symbols, drawings and real objects to help them with the planning process.

Templates and fact sheets for young people

  • About me: questionnaire - describes the purpose of the questionnaire and includes questionnaire template
  • Personal action plan - describes what a 'Personal Action Plan' is and inlcudes the personal action plan template and 'Developing a Plan' information
  • Moving on checklist - describes the purpose of the 'Moving On' checklist and includes 'Developing a Plan' information and 'Moving On' checklist
  • Sample invitation letter - describes the purpose of two letters and includes a template for a letter to Parents/Guardians and a template for the young person's letter to team members
  • Young Person's Fact Sheet - describes the purpose of the young person's fact sheets and includes three individual factsheets - 'Planning for My Future: My Guide', 'Making Choices' and 'Rights and Responsibilities'
