Student and family details

This information is for parents and guardians enrolling a child in a Tasmanian Government school. When enrolling the student, you'll be asked to supply the following information about the student and yourself.

About the student

  • Evidence of date of birth and identity. A Birth Certificate is usually the easiest to provide, but the school can advise on other forms of evidence. Copies cannot be accepted.
  • Proof of independent status if seeking to enrol without the signature of a parent or another person having the student's care or control.
  • Proof of your child's immunisation status. Evidence is only needed at the first State school or college at which enrolled and after that updates can be advised as they occur.
  • Your child's name, residential and postal addresses, previous schools attended and the approximate date of leaving the last school.
  • Other surnames or given names that currently identify the student or have been used in the past to identify the student.
  • Emergency contacts.
  • Name of any medical condition (parents or the student's doctor may provide separate written advice of medical details or request in writing that staff administer student medication).
  • Medical practitioner or clinic name, address and telephone number.
  • Country of birth.
  • Any language spoken at home other than English.
  • Whether the student is of Aboriginal or Torres Strait Islander origin.

About the parents or persons having care or control of the enrolling student

This information is not required for students who are independent or aged 18 and above.

If the student resides with both parents/guardians this information is to be collected from both parents/guardians.

If the parents/guardians are separated this information is to be collected from the parent or guardian with whom the student resides. However, if the separated parents/guardians share care and control of the student the information is to be collected from both parents/guardians.

  • Surname or family name, given name(s) and title
  • Sex
  • Address
  • Occupation group selected from the list specified on the Student Enrolment Application Form
  • Country of birth
  • School education and qualifications
  • Language spoken at home other than English
  • Relationship to the enrolling student

Other information

  • Any Court parenting orders concerning residence, contact, child maintenance, education, health, or other specific issues relating to the enrolling student.
  • Consent or non-consent to the publication of photographs and student work.
