The immunisation status must be recorded for all students under 18 years of age currently enrolled in a Tasmanian government school. Students do not have to be immunised in order to attend school, but their immunisation status must be noted.
If a child is not vaccinated, he or she may be excluded from school if there is an outbreak of a vaccine preventable disease at the school.
Parents and guardians will need to state whether or not their child has been immunised. If they fill out the enrolment form and state that their child has received particular vaccinations, they are required to provide evidence to prove this.
This is usually in the form of either:
- the child's health record book
- signed documentation from the child's doctor on letterhead
- signed documentation from the council on letterhead
- a printout of immunisation history recorded by the Australian Childhood Immunisation Register, Medicare Australia. Phone 1800 653 809
If proof is not available, a statutory declaration form can be obtained from your school or from the Department of Health and Human Services, Tasmania: Phone 1800 671 738
Statutory Declaration A states that the parent or guardian believes the child has been immunised, but does not have any documentation available to prove this.

Immunisation is not required in order to enrol in a Tasmanian government school. However, the immunisation status must be noted on the enrolment form. If the parents or guardians object to immunisation and choose not to have their children immunised, they should provide a statutory declaration to the school.
Statutory Declaration B states that the parent or guardian has a conscientious objection to immunisation.

Immunisation status will still need to be provided on the enrolment form along with information in writing confirming the reasons why the student cannot be immunised.

If a student receives additional vaccinations, this information can be updated on the validation form that is sent out to parents and guardians each year. Proof of immunisation is not required.

Students over 18 are not required to provide evidence of immunisation, however this information is still beneficial and is to be recorded if provided.

Refugees may not have vaccination documentation. Parents should be encouraged to visit a General Practitioner for assessment and to obtain the necessary documents.
